Change of details
If you change name, address or telephone number, please let us know as soon as possible. This helps us to keep our records up to date.
From time to time, we may ask you to complete a Medical History Questionnaire to assist in our diagnosis and treatment.
We also need to know of any medication you may be taking. This is of course totally confidential.
We operate a practice complaint procedure which complies with national criteria. Please ask at reception for details.
Following NHS guidelines, patients who are violent or abusive to any dentist, member of staff or persons present on the practice premises will lose their right to treatment at this practice and will be reported to the NHS and police if appropriate.
All patients’ details and treatments are confidential. Details are not released to a third party without the patient’s consent. Patients are able to request sight of their records on application to the practice.
We are registered with the Data Protection Agency and comply with their regulations.

Facilities for the disabled
Unfortunately, our practice does not have wheelchair access as all our surgeries are situated on the first and second floor. The practice building is a listed building, which does not allow for any structural changes, making installing a lift impossible. Alternative surgeries can be suggested or we will contact our local PCT to find a practice with ground floor access. A member of the dental team can help patients with limited mobility. We have also installed handrails to help patients negotiate the stairs.
